Strategies to Maintain HIV Viral Suppression Among State Prison Inmates Released to the Community
This NOFO aims to develop effective, sustainable, and replicable program models that support continuity of HIV care for PWH released from state prisons into the community.
Programs will emphasize linkage to and retention in community HIV care, adherence to HIV treatment, and suppression of
HIV viral load.
Programs should also identify PWH and HIV-negative persons at risk for HIV among sex and drug-injection partners and associates of program participants and link them to care.

Additional Information of Eligibility:
To be considered responsive, applicants must be able to document all of the following: Complete laboratory reporting of all HIV viral load and CD4 data to the state health department.
There are at least 250 HIV-positive persons confined in the state prison system.
These data should be based upon the most recent available information compiled by the Department of Justice/Bureau of Justice Statistics.

Release information for HIV-positive prison inmates can be based upon calculations using general release data collected by the Department of Justice/Bureau of Justice Statistics.

The letter of support from the state health department should include a clear commitment to the overall goals of the project from both the HIV surveillance and prevention program units.
Applicants should provide a one-page brief that summarizes how they meet the four elements of responsiveness.
The brief should include a list of all agencies and management personnel who have signed letters of support that are attached to the application.
The brief should also include page numbers referencing where more information can be found in the application narrative.
Applicants should upload this document (file name: ‘Summary of Responsiveness’) to www.grants.gov as a component of their application package.
Applicants that have not provided evidence of all four elements will be deemed non-responsive and will not be passed along for further review.
Applicants requesting more than the award ceiling will be considered non-responsive.
