Additional Information of Eligibility:
Eligibility for this NOFO is limited to States and, in consultation with the State health authority, political subdivisions of States that have the authority to conduct, and are directly funded by CDC to conduct, HIV surveillance.

This program is authorized under under section 318 (c-d) of the Public Health Service Act (42 U.S.C.

Section 247c (c-d)), as amended.

CDC will consider any application requesting an award higher than the award ceiling as nonresponsive and it will receive no further review.

To be considered responsive, applicants must be able to document all of the following: Complete access to the state department of health HIV surveillance data system for the implementation of Data-to-Care program requirements.

The existence of laws and regulations that require complete laboratory reporting of all HIV viral load and CD4 data to the state health department.

There are at least 500 HIV-positive persons confined in the state prison system.

These data should be based upon the most recent available information compiled by the Department of Justice/Bureau of Justice Statistics.

Release information for HIV-positive prison inmates can be based upon calculations using general release data collected by the Department of Justice/Bureau of Justice Statistics.

Applicants should provide a one-page brief that summarizes how they meet the five elements of responsiveness.

This one-page brief should include a summary list of all entities providing a letter of support.

The brief should also include page numbers referencing where more information can be found in the application narrative.

Applicants should upload this document (file name: ‘Summary of Eligibility’) to www.grants.gov as a component of their application package.

Applicants that have not provided evidence of all five elements will be deemed non-responsive and will not be passed along for further review.
