Prevention and Control of Chronic Disease and Associated Risk Factors in the U.S. Affiliated Pacific Islands, U.S. Virgin Islands, and Puerto Rico
This NOFO works to prevent and control tobacco use, diabetes, and cardiovascular disease in the USAPIs, the U.
S.
Virgin Islands, and Puerto Rico.
This funding opportunity is comprised of two components – the Core Component and an optional Oral Health Component.
/>Core Component:
This competitive component is required of all applicants and will support the implementation and evaluation of a core set of evidence-based strategies designed to address tobacco use and secondhand smoke exposure, diabetes prevention and management, and cardiovascular disease prevention and management.
Applicants must propose a comprehensive work plan that addresses each of these areas in alignment with the four strategies indicated in the NOFO.
Oral Health Component:
This competitive component is optional and will support the implementation and evaluation of a small set of activities designed to improve oral health capacity and better address the oral health needs of Island communities.
All applicants must apply for the Core Component in order to be considered for the optional Oral Health Component via a single application that clearly identifies the Core and Oral Health Components.
Only successful applicants under the Core Component will be eligible for Oral Health Component funding.
Final funding amounts will depend upon the quality of the application, regardless of whether an applicant intends to support implementation of the Oral Health Component.

Additional Information of Eligibility:
If the applicant is other than a Department/Ministry of Health in the specified jurisdiction, the applicant must submit a memorandum of agreement/memorandum of understanding with the Department/Ministry of Health in the jurisdiction for which it is applying.
CDC will need certain surveillance and health system data from applicants in order to ensure performance measures are being met for the benefit of achieving public heath results and hence, applicants must demonstrate they have the authority to submit the data to CDC and will have access to the data.
The MOU/MOA should specify that the Department/Ministry of Health will provide the applicant access to surveillance and health system data, has granted the applicant the authority to submit the data, and will collaborate with the applicant on this NOFO to ensure complete data are submitted to CDC according to established guidelines and processes.
The MOU/MOA must be provided in an attachment labeled “MOU/MOA” when submitting via www.grants.gov.

CDC will consider any application requesting an award higher than the ceiling amount of $1,400,000 to be non-responsive Applicants must provide proof of ability to serve populations in one of the jurisdictions listed below: Commonwealth of the Northern Mariana Islands (CNMI), American Samoa, Guam, Federated States of Micronesia (FSM), Republic of the Marshall Islands (RMI), Republic of Palau, U. S. Virgin Islands, Puerto Rico, Evidence of ability may include documentation that the applicant is a territorial government or a bona fide agent in the Commonwealth of the Northern Mariana Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, the Republic of Palau, U. S. Virgin Islands, or Puerto Rico.
Documentation must be provided in attachment labeled “Evidence of Ability” when submitting via www.grants.gov.
A bona fide agent is an agency/organization identified by the state as eligible to submit an application under the state eligibility in lieu of a state application.
If applying as a bona fide agent of a state or local government, a legal, binding agreement from the state or local government as documentation of the status is required.
Attach with “Other Attachment Forms” when submitting via www.grants.gov Applications that do not comply with these requirements will be determined non-responsive and will not receive further review.
